What the position looks like:

You can make a life-changing impact on people from all walks of life. Work one-on-one with your patients in the comfort of where they call home, which is primarily senior living settings. This allows you to develop deeper relationships and work in a more personal setting than a traditional medical office. Additionally, you will practice with more independence and autonomy than in traditional roles, with the flexibility to build your own schedule.

Provide one-on-one care to improve performance of activities of daily living for elderly, critically ill, disabled, those recovering from a surgery, injury, or accident, or a combination. Review physician referrals, assess conditions, and develop an individualized plan of care designed to meet patient-centered goals. Coordinate care with an interdisciplinary team by documenting patient condition, progress, and education provided to patient and family/caregiver. Supervise delegation of work performed by COTA.

What you will need:

· Current active license in good standing with the state of employment

· CPR certification, we will provide if needed

· Positive attitude for learning and growing

· Full time between 32 – 40 hours per week
