What the position looks like:

Provide life-enhancing care for people from all walks of life based on their unique needs. Work one-on-one with your patients in the comfort of where they call home. This allows you to develop deeper relationships and work in a more personal setting than a traditional medical office. Additionally, you will practice with more independence and autonomy than in traditional nursing roles.

Provide one-on-one care that focuses on comfort and quality of life at the end of life. Independent work with the support of an experienced team. Discuss and explain a patient’s prognosis. Provide education to patient families about the disease process, hospice philosophy, nutrition, medication, and skin care. Assess patient needs and develop an individualized plan of care with the physician. Coordinate and oversee the care plan with the interdisciplinary team as the designated case manager. Assess vitals, pain, wounds, medications, catheter care, and more. Check for any safety hazards and teach family/caregivers about medication indications and side effects.
