What the position looks like:
Apply nursing skills where the patient calls home – in the senior living setting. Oversight of both home health and hospice healthcare needs within the aging population. Independent work with the support of an experienced team. Ability to practice at the top of your license. Identify patient needs with the help of an interdisciplinary team. Provide patient assessments. Design and implement care plans. Assist with setting and achieving patient health goals. Connect patients with resources. Participates in supporting patient care in on-call rotation.
What you will need:
Current active license in good standing with the state of employment
CPR certification, we will provide if needed
