What the role looks like:
- Deliver skilled nursing care including wound care, post-op recovery, cardiopulmonary assessment, diabetic education, and disease management
- Provide compassionate end-of-life support, including family education and hospice philosophy care
- Assess patients, design and implement individualized care plans, and coordinate services
- Collaborate with physicians and interdisciplinary teams to deliver quality, holistic care
- Engage with families to ensure comfort, understanding, and support throughout the care journey
What you will need:
- Current RN license in good standing with the state of Minnesota
- CPR certification (we will provide if needed)
- Reliable transportation and valid driver’s license
- A compassionate, team-focused attitude with a desire to grow professionally
