Physical Therapist (PT) – A Life Changing Impact in Community Health
Hermantown, Minnesota
Make a life-changing impact on people from all walks of life. In Home Health, you work one-on-one with your patients in the comfort of wherever they call home. Additionally, you will practice with more independence and autonomy than in traditional roles with the flexibility to build your own schedule.
At CaringEdge, we prioritize providing modern, challenging, and evidenced -based care to restore functionality by improving movement and independence for our primarily geriatric patient population. Our patients are critically ill, disabled, those recovering from a surgery, injury, or accident, or a combination. You would work with patients with primary diagnoses including neurological, cardiac, pulmonary, chronic illness, chronic pain, oncology, and more! The PT in this position will assist with reviewing physician referrals, assess conditions, and develop an individualized plan of care designed to meet patient-centered goals. Coordinate care with an interdisciplinary team by documenting patient condition, progress, and education provided to patient and family/caregiver. Supervise delegation of work performed by PTA.
