CaringEdge Health Services is seeking a compassionate and independent Physical Therapist (PT) to join our home health team in Hermantown, MN. Enjoy the opportunity to serve a vibrant and deserving aging population while working one-on-one with patients in their homes throughout Hermantown, Duluth, Proctor, Cloquet, and nearby communities.
This full-time position provides the freedom to build your own schedule, the autonomy to work independently, and the chance to deliver high-impact, patient-centered care in a rewarding environment.
What You’ll Do:
- Provide personalized in-home therapy services with a focus on improving function and independence
- Review referrals, assess patient needs, and create individualized treatment plans
- Work with a geriatric population with conditions such as neurological disorders, cardiac and pulmonary disease, chronic illness, post-surgical rehab, and more
- Collaborate with an interdisciplinary team and supervise Physical Therapist Assistants (PTAs)
- Offer education and support to patients and caregivers to maximize outcomes
What You Will Need:
- Active Physical Therapist license in good standing in Minnesota
- CPR certification (provided if needed)
- A positive attitude and a passion for learning and serving others
