Description
Care for the social and emotional needs of the aging population through end of life. Collaborate with the interdisciplinary team. Make a lasting difference in patients and families lives. Counsel patients and families on stress management. Provide crisis intervention. Design and implement patient treatment plans. Attend regularly scheduled interdisciplinary group meetings. Assist the team in recognizing the psychological stress of a terminal illness. Identify and connect patients with community resources.
What you will need:
· Licensed by the state of employment, required
· Bachelor required, Master’s preferred
· CPR certification, we will provide if needed
· Positive attitude for learning and growing
