OneSource Home Health is currently seeking a compassionate and skilled Licensed Practical Nurse (LPN) to provide high-quality home health and hospice care in Idaho Falls, ID. This full-time opportunity allows you to work independently, build deeper patient relationships, and contribute meaningfully to your community through one-on-one care.
What You’ll Do:
- Deliver personalized nursing care to patients in the comfort of their homes
- Practice independently in collaboration with an interdisciplinary care team
- Apply your skills in wound care, medication administration, chronic disease support, and more
- Support patients through hospice and palliative care, promoting comfort and dignity
- Participate in patient care planning and contribute to meaningful outcomes
What You Will Need:
- Active LPN license in good standing in the state of Idaho
- CPR certification (provided if needed)
- A positive, professional attitude and desire to grow in a supportive environment
